Job summary

Yellowstone Boys and Girls Ranch is seeking a Targeted Case Manager to help youth and families access services across Montana. You will advocate, assess needs, refer to supports, and collaborate with providers to create and monitor service plans.

Responsibilities include coordinating with schools and community partners, documenting in EMR, and leading treatment team meetings. A Bachelor's in human services and background checks are required; starting pay ranges from $20.38 to $22.94 per hour,

Qualifications

• Bachelor’s degree in human services or related field.
• Pass background checks as required by Montana.
• Ability to relate to youth and families therapeutically.
• Must be 21 years or older.

Responsibilities

• Advocate for youth and families to obtain services, create and monitor service plans.
• Coordinate with service providers and professionals to meet needs.
• Identify community resources to support youth and families.
• Develop and lead treatment team meetings with youth and family.
• Complete documentation per COA, Montana ARM's, and YBGR policies.
