Job summary

Care Lync is hiring a Support Coordinator to empower adults with intellectual, developmental, and physical disabilities, and aging adults, to access needed services and maintain independence. You will conduct person-centered assessments, develop ISP plans, advocate for participants, and coordinate with healthcare, behavioral, and community resources to ensure quality care and progress toward goals.

Qualifications

• Bachelor's degree in social work, psychology, human services, or related field.
• Strong organizational, communication, and advocacy skills.
• Comfort working independently and traveling up to 50% to meet with participants in homes and communities.

Responsibilities

• Conduct person-centered assessments to identify needs, preferences, and goals.
• Develop, implement, and update Individual Support Plans (ISP) to ensure appropriate services.
• Provide participants with choice and advocacy to make informed provider and care decisions.
• Coordinate with medical professionals, behavioral specialists, family members, and community resources to deliver high-quality care.
• Monitor and evaluate services to ensure safety, satisfaction, and progress toward independence.
• Support both individuals with disabilities and aging population in accessing community resources.
• Maintain documentation, confidentiality, and compliance with program and ethical standards.
• Participate in ongoing training and professional development to strengthen skills.
