Job summary

Care Lync is seeking a dedicated Support Coordinator to assist adults with disabilities and aging individuals in navigating access to housing, medical care, and community resources. The role emphasizes person-centered assessment, ISP development, and advocacy to empower participants.

Travel to participant homes and communities is required. The position offers professional development and mentorship within a collaborative team, with a focus on compassionate, client-centered care and ethical

Qualifications

Bachelor's degree in Social Work, Psychology, Human Services, or related field.
Strong organizational, communication, and advocacy skills.
Comfort working independently and traveling up to 50% to meet with participants in homes and communities.

Responsibilities

Conduct person-centered assessments to identify needs, preferences, and life goals.
Develop, implement, and update Individual Support Plans (ISP) to ensure services.
Provide participants with choice and advocacy in selecting providers and care options.
Coordinate with medical professionals, behavioral specialists, family, and community resources for seamless care.
Monitor and evaluate services to ensure safety, satisfaction, and progress toward independence.
Support individuals with disabilities and aging population in accessing community resources.
Maintain documentation, confidentiality, and compliance with program and ethics.
Participate in ongoing training and professional development to strengthen skills.
