Job summary

Care Lync is seeking a Support Coordinator to help adults with disabilities and the aging population access housing, medical care, and community resources, while promoting independence.

You will conduct person-centered assessments, develop Individual Support Plans, and coordinate with a network of professionals to ensure high-quality, client-centered services. Travel to participant homes will be required.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational, communication, and advocacy abilities.
• Comfort with travel to meet participants in homes and communities.

Responsibilities

• Conduct person-centered assessments to identify needs, preferences, and goals.
• Develop, implement, and update Individual Support Plans (ISP) for services.
• Provide choice and advocacy to help participants select providers and care options.
• Coordinate with medical professionals, behavioral specialists, family, and community resources.
• Monitor services to ensure safety, satisfaction, and progress toward independence.
• Assist adults with disabilities and seniors in accessing community resources.
• Maintain documentation, confidentiality, and program compliance.
• Participate in ongoing training and professional development.
