Job summary

Qualifications

• BS/BA degree required.
• Experience with Microsoft Office and state-maintained databases.
• Ability to perform basic arithmetic and data entry.
• Clean criminal background and state clearances.
• Valid driver’s license with current insurance and vehicle registration.

Responsibilities

• Complete assessments to identify participant needs.
• Develop a Person-Centered Service Plan for each participant.
• Coordinate with providers to implement the ISP.
• Maintain accurate documentation and reporting.
• Travel up to 50% to participant sites.
