Job summary

Project FIND in New York City is seeking a Social Work Case Manager to help older adults access services, benefits, and entitlements while supporting independence and quality of life. You will manage a caseload of 65+ clients, working to develop trusting relationships and collaborative problem-solving.

Responsibilities include intake screening, home assessments, care planning, and coordinating with providers to deliver culturally sensitive services.

Qualifications

MSW preferred or related master’s degree in a relevant field.
Bachelor’s degree with at least two years of aging-related work experience.
Associates degree with four+ years of casework in a community setting.
Demonstrate cultural competence and sensitivity to diverse communities.
Ability to multi-task and prioritize effectively.
Strong verbal and written communication and customer service skills.

Responsibilities

Screen intake and assess client needs to determine services required.
Conduct in-home assessments to determine eligibility for services.
Develop care plans and coordinate with providers and resources.
Maintain complete and accurate case files and prepare reports.
Collaborate with staff and partners; participate in outreach as needed.
Attend required training and ensure client needs are addressed with sensitivity.
