Job summary

JASA is seeking a Part-Time Case Manager to support older adults and their families in New York City. You will conduct assessments, develop care plans, and advocate for benefits while coordinating services like homecare, transportation, and meals on wheels.

You’ll need a BSW/BA/BS or equivalent, strong computer and record-keeping skills, and the ability to travel to clients. Spanish helpful; strong communication and cultural awareness are essential.

Qualifications

• Graduate degree required: BSW/BA/BS or equivalent.
• Proficiency with standard software and record-keeping.
• Ability to read, write, and speak English; Spanish helpful.
• Ability to travel to clients using mass transit and stairs in residences.
• Understanding and appreciation of diverse communities.

Responsibilities

• Manage all aspects of social casework from assessment to monitoring.
• Develop and implement personalized care plans for older adults.
• Advocate for entitlements and connect clients to benefits.
• Coordinate homecare, meals, transportation, and related services.
• Identify signs of abuse or exploitation and refer to Protective Services.
• Provide group counseling and community education; represent the agency.
• Maintain accurate case records and participate in trainings.
