Job summary

Project FIND is seeking a Social Worker to serve older adults at Hargrave House in New York. The role involves detailed assessments, case management, counseling, housing assistance, and collaboration with tenants and staff to help residents live independently.

You will conduct intake, develop goal-oriented service plans, provide group work and crisis intervention, and document progress to ensure timely services for clients and families.

Qualifications

• Bachelor's degree in human services or related field required; MSW/LMSW preferred.
• Experience working with older adult populations and families preferred.
• Bilingual Spanish fluency preferred.
• Strong computer skills, including Microsoft Office.
• Ability to provide accurate and timely documentation.
• Time management skills to coordinate multiple responsibilities and services.
• Ability to work independently and as part of a team, in both home and office environments.
• Maintain client and agency confidentiality.
• Excellent interpersonal, public speaking, and written communication skills.

Responsibilities

• Conduct intake and assessment screening, addressing mental/physical health needs and financial eligibility; visit homes/offices.
• Provide counseling, education on health supports, long-term planning, and benefits assistance.
• Support tenants to remain permanently housed and safe.
• Assist with income affidavits, housing applications, and recertifications; collect rent as needed.
• Develop training events to educate clients on self-care, health, and legal planning.
• Lead one or more support groups with psychoeducation on aging in place, health and wellness, and self-care.
• Create community events to promote wellness and socialization.
• Provide conflict resolution and address tenant concerns requiring intervention.
• Maintain timely documentation and progress notes.
• May supervise or train paraprofessional staff, including students.
• Perform other related tasks as requested.
