Qualifications

• Knowledge of HRA benefits, SNAP, Medicaid, and other public assistance programs.
• Bilingual ability in Spanish or another language.

Responsibilities

• Provide comprehensive case management and social service support to senior residents.
• Assist residents with navigating HRA systems and applying for SNAP, Medicaid, and recertifications.
• Connect residents with community resources and outside service providers based on needs.
• Develop and facilitate educational, recreational, and wellness groups for healthy aging.
• Conduct assessments and maintain accurate case notes and documentation.
• Advocate on behalf of residents and collaborate with staff and partners.
• Meet with new residents and assist with intake forms.
