Job summary

Qualifications

• Ability to travel to clients' homes via mass transit.

Responsibilities

• Conduct initial assessment and develop written case plans.
• Provide individual and family counseling and crisis intervention.
• Advocate for entitlements (Medicare, Medicaid, SNAP) and assist with applications.
• Coordinate direct services (homecare, transportation, meals on wheels, legal aid, psychiatric exams).
• Monitor risk, reassess, and maintain accurate case records.
