Responsibilities

• Participate in staff meetings, case conferences, supervision, training, and outreach to increase participation.
• Ensure timely response and follow-up to referrals.
• Help participants access social services, wellness education, and recreation activities.
• Develop individualized service plans with goals and regular reassessment.
• Maintain a directory of community resources and partner agencies.
• Submit weekly dashboards and service logs with outcomes.
