Abound Health is seeking a passionate Qualified Professional (QP) to advocate for individuals with IDD and help them achieve meaningful goals. You will develop and monitor service plans, supervise staff, and participate in ISP meetings with families and care teams.

The role emphasizes compliance, quality care, and strong community partnerships. We offer competitive benefits, begin the health plan on day 60, and a 12-month 401(k) eligibility with a bonus potential of up to 25% biannually.

Qualificações

• Bachelor's degree in a related human services field and 2 years of full-time IDD experience; or Bachelor's degree in an unrelated field and 4 years of full-time IDD experience; or Master's degree in a related human services field and 1 year of full-time IDD experience.

Responsabilidades

• Advocating for people we serve and helping them achieve their goals.
• Developing and monitoring individualized service plans.
• Providing guidance and supervision to direct support staff.
• Participating in ISP meetings and collaborating with families and care teams.
• Ensuring compliance, documentation accuracy, and quality service delivery.
• Building strong community partnerships and serving as a resource for stakeholders.
• Passionate about supporting individuals with IDD.
• Strong communicator who builds relationships easily.
• Organized, detail-oriented, and able to manage competing priorities.
• Knowledgeable of Medicaid, State Funded Services, and documentation requirements.
• Committed to delivering high-quality, person-centered care.
