Job summary

Abound Health is seeking a Qualified Professional to support and advocate for Service Recipients receiving Innovations Waiver or State Funded Services. You’ll be at the heart of helping individuals live meaningful lives, ensuring they receive the highest level of care and support.

This role requires a Bachelor’s or Master’s in a related field with IDD experience, a detail-oriented approach, and the ability to collaborate with teams.

Qualifications

• Bachelor’s degree in a related human service field with 2 years of full-time IDD experience.
• Bachelor’s degree in an unrelated field with 4 years of full-time IDD experience.
• Master’s degree in a related human service field with 1 year of full-time IDD experience.

Responsibilities

• Develop individualized short-term goals and interventions.
• Conduct clinical and employee supervisory visits, ensuring the highest standards of care and compliance.
• Provide ongoing development plans and annual performance evaluations for direct care staff.
• Participate and advocate for recipients in their ISP meetings, ensuring their voices are heard.
• Monitor billing, maintain accurate documentation, and resolve grievances swiftly and compassionately.
• Represent Abound Health at community events, spreading awareness and making connections.
