Job summary

RHA Health Services, LLC is hiring an IDD Qualified Professional to coordinate and monitor person-centered plans for individuals with intellectual and developmental disabilities. The role involves supervising Group Home Managers and Direct Care staff, conducting interviews to identify service needs, and aligning programs with person-centered outcomes.

You will train staff across residential and day locations, complete evaluations, and ensure quality assurance.

Qualifications

• Bachelor’s degree in Social Work, Social Services, Human Services or Special Education required
• Two years experience in human services or four years in other fields; license in a human service profession is acceptable
• Master’s degree in a Human Service field preferred

Responsibilities

• Coordinate and monitor person-centered plans and outcomes with an interdisciplinary team
• Supervise Group Home Managers and Direct Care Associates; visit sites to train staff
• Complete incident reports and performance evaluations
• Identify and document people’s goals and service needs through personal outcome interviews
• Lead periodic reviews and adjust plans based on feedback and assessments
