Qualifications

• MD or DO in Internal Medicine, Family Medicine, or Geriatrics.

• Experience with value-based care and care coordination is preferred.

Responsibilities

• Provide comprehensive care for older adults with acute and chronic illnesses.
• Lead a care team and coordinate with hospitalists, APPs, and other providers.
• Engage in recruiting patients and providers to grow the center and market prominence.
