Job summary

Compassus in Savannah seeks a Palliative Care Nurse Practitioner to deliver patient-centered palliative care, collaborate with PCPs and the IDT, and support families with education and symptom management.

Qualifications

• Master’s degree in Nursing required.
• Minimum of five years nursing experience; oncology, hospice, geriatrics, or ICU preferred.
• Active and unencumbered Nurse Practitioner license.

Responsibilities

• Performs routine and urgent assessments; collaborates with Medical Director and IDT.
• Educates patients and families regarding care goals, disease process, and prognosis.
• Maintains accurate patient records and supports care coordination and documentation.
