Job summary

Compassus is seeking a Palliative Care Nurse Practitioner to provide comprehensive palliative care, education, and coordination across locations. You will collaborate with the Palliative Care Physician and the Medical Director, guiding patient and family goals of care while ensuring compliance with practice standards.

The role requires an NP or CNS license, board certification in hospice/palliative care preferred, and the ability to travel regionally.

Qualifications

• Active CNS or NP license in state of employment.
• NP certification in Adult, Family, Acute, or Geriatric.
• Active NPI number and DEA certification if allowed.
• Hospice/palliative care board certification preferred but not required.

Responsibilities

• Perform routine and urgent assessments; ensure documentation within CMS guidelines.
• Provide palliative care education, symptom management, and care planning.
• Collaborate with palliative care physician and IDT; coordinate care and referrals.
• Complete billing, coding, and medical records within 24 hours of visits.
• Travel within local region; participate in after-hours and weekend on-call as required.
