Job summary

Nightingale Respite Care LLC in Michigan seeks an experienced Service Coordinator to support our Medicaid- and VA-funded home care operations. You will own client and caregiver coordination, ensure service coverage, and maintain accurate documentation.

The role requires strong organizational skills, proactive problem solving, and the ability to build relationships with clients, families, referral partners, and case managers. EVV oversight and compliance are part of daily duties.

Qualifications

• Minimum of 1 year direct Medicaid-funded home care experience.
• Experience with Medicaid service coordination including authorizations, hours, documentation, and case manager communication.
• Caregiver scheduling and client service management experience.
• Familiarity with EVV and home care documentation standards.
• Strong organizational, communication, and problem-solving skills.
• Ability to work independently, manage priorities, and follow through.

Responsibilities

• Coordinate in-home care for Medicaid- and VA-funded clients.
• Manage caregiver schedules, staffing, and service coverage.
• Assist with caregiver recruitment, onboarding, and retention.
• Maintain regular communication with clients, families, caregivers, and referral partners.
• Respond promptly to staffing challenges, service interruptions, and client concerns.
• Promote continuity of care and client satisfaction.
• Monitor EVV, visit documentation, and scheduling accuracy.
• Utilize home care management software to oversee daily operations.
• Identify and resolve documentation concerns and compliance issues.
• Track unstaffed hours and caregiver availability.
