Qualifications

• Minimum 1 year of direct Medicaid-funded home care experience.
• Experience with Medicaid service coordination, including authorizations, approved hours, documentation, and case-manager communications.
• Experience with caregiver scheduling, staffing coordination, and client service management.
• Familiarity with Medicaid EVV and home care documentation standards.
• Strong organizational, communication, and problem-solving skills.

Responsibilities

• Coordinate in-home care services for Medicaid-funded and VA-funded clients.
• Manage caregiver schedules, staffing assignments, and service coverage.
• Assist with caregiver recruitment, onboarding, and retention.
• Maintain regular communication with clients, families, caregivers, and referral partners.
• Respond promptly to staffing challenges, service interruptions, and client concerns.
• Promote continuity of care and exceptional client satisfaction.
