Durham Housing Authority is seeking a passionate Jobs Plus Case Manager to support residents in achieving economic independence. The role provides comprehensive case management, employment support, and service coordination for participants in the HUD-funded program.

You will recruit participants, conduct assessments, develop individualized employment plans, and collaborate with community partners to improve outcomes.

Formación

• Bachelor's degree in Social Work, Sociology, Human Services, or a related field.
• Minimum of two years of case management or workforce development experience.
• Experience with underserved populations and barriers to employment preferred.
• Equivalent education with relevant experience may be considered.

Responsabilidades

• Conduct eligibility screenings, assessments, and intake activities.
• Develop individualized employment plans and service plans.
• Provide intensive case management and ongoing support.
• Monitor progress and adjust service strategies as needed.
• Respond to crises and connect to resources and supports.
• Connect participants with job training and employment resources.
• Identify and address barriers to employment (childcare, transportation, etc.).
• Provide budgeting and life-skills coaching.
• Support job placement and early retention in employment.
• Conduct outreach activities and community recruitment.
• Collaborate with employers and community partners.
• Develop and facilitate Job Clubs, mentoring, and workshops.
• Maintain accurate case files and program documentation.
• Assist with data reporting and grant compliance.
• Supervise support staff and consultant contracts where assigned.
