Job summary

Housing Forward seeks a Health Case Manager Supervisor to coach and supervise case managers, strengthen housing and health partnerships, and monitor program metrics. The role includes program planning, work plans, process improvements, and staff assignments to optimize service delivery.

The incumbent will oversee HMIS data entry, maintain case records, and collaborate with staff and partners to refine strategies and achieve program goals.

Qualifications

• MSW or related master’s degree required.
• Bachelor’s degree with 4 years of experience including lived experience and 3 years supervisory experience in lieu of a master will be considered.
• Demonstrates ability to supervise a team with varied responsibilities.

Responsibilities

• Provide supervision of case managers and monitor progress toward program scopes/objectives.
• Oversee data entry into HMIS and Client Track; maintain case files and documentation.
• Review initial assessments and ensure timely re-certifications and service plans.
• Conduct monthly quality assurance activities and update service planning and task alignment.
• Manage health and case management contracts and budgets; collaborate with partners to improve services.
