Qualifications

• Bachelor’s degree in human services (or related field) with one year of experience.

Responsibilities

• Carry a caseload of 25 clients.
• Interview and assess each client within 24 hours of admission.
• Conduct intakes and assess residents’ needs; coordinate referrals.
• Develop individualized service plans with short- and long-term goals; review bi-weekly.
• Monitor goal progress with weekly appointments; ILP reviewed bi-weekly.
• Maintain orderly files and document missed appointments; arrange makeups.
• Work with shelter staff to enforce rules; hold conferences for rule violations.
• Advocate with agencies to minimize barriers to needed services.
• Follow up with referral sources to ensure timely access to services.
• Conduct apartment inspections to expedite re-housing.
• Coach clients on job prep, housing availability, and social services.
• Represent CP Unlimited at coalitions and committees as assigned.
