Job summary

HousingPlus is looking for a dedicated Case Manager to work in supportive housing programs in Brooklyn, New York. The position involves supporting women and families who have faced chronic homelessness and related challenges. Responsibilities include case management, conducting assessments, and collaborating with community services to achieve housing stability.

The ideal candidate will have a Bachelor's degree in Human Services or a related field, coupled with at least 2 years of relevant experience. Strong organizational and communication skills are essential.

Qualifications

• Minimum of 2 years’ experience with families addressing homelessness and mental health issues.
• Proficient computer skills, including Microsoft applications and Google Suite.
• Must be able to walk a minimum distance of 4 city blocks and climb stairs.

Responsibilities

• Conduct initial assessments and develop Individual Service Plans (ISP).
• Provide case management and collaborate with community service providers.
• Maintain organized tenant case files and documentation.
