Qualifications

• Qualified Professional must have a four-year degree in a human service field and two years of experience or a four-year degree in other fields with four years of experience or hold a license in a human service profession.
• Graduate of a college or university with a Master’s degree in a Human Service field preferred.

Responsibilities

• Serve as the center of the interdisciplinary service team to guide development of person-centered plans.
• Complete incident reports and employee performance evaluations.
• Supervise, integrate and coordinate person-centered plans and monitor progress toward outcomes.
• Initiate periodic reviews and solicit feedback as needs and goals change.
• Review assessments and observations to help people meet their full potential.
• Oversee residential activities to ensure people are healthy and safe.
• Visit vocational centers and residential homes to assess plan implementation.
• Report to the Administrator and coordinate with colleagues to improve procedures and workflow.
• Assist with recruiting, hiring, training and, when needed, dismissal of staff.
