Job summary

Indiana Professional Management Group, Inc. is seeking a Case Manager for a full-time, hybrid role serving Monroe County, Indiana.

The position blends remote work from a home office with in-person meetings within the county and surrounding areas, requiring residence within Monroe County and regular travel for community meetings. You will educate about Medicaid waivers, tailor individual support plans, connect individuals to needed services, and lead team meetings with care providers to review

Qualifications

• Bachelor's degree in psychology, sociology, social work, or related field.
• Alternatively, a Bachelor's in any field plus 1 year direct experience with individuals with IDD or 2 years with aging population.
• Valid driver’s license, car insurance, and reliable transportation.
• Ability to pass background checks.
• Home office with computer, internet and smartphone.

Responsibilities

• Provide information and education about Medicaid waiver processes.
• Create individual support plans based on individuals' strengths, needs, goals and desires.
• Help individuals access services and supports to meet goals.
• Facilitate regular community-based team meetings to review plans of care and collaborate with providers.
