Job summary

Indiana Professional Management Group (IPMG) is seeking a full-time Case Manager for Monroe County with a hybrid schedule—remote work from home and travel for in-person community meetings several times a week.

Applicants must reside within Monroe County, Indiana, and be able to travel to surrounding areas as needed. A Bachelor’s degree in a human services field or equivalent experience is required, along with a valid driver’s license and a reliable home office setup.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Bachelor's degree in an unrelated field with a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
