Indiana Professional Management Group is seeking a Case Manager to support individuals across Marion County. The hybrid role blends remote work with in-person community meetings, requiring residency within Marion County, Indiana, and the ability to travel across the county.

Responsibilities include educating clients on Medicaid waivers, creating tailored support plans, and coordinating with providers to meet goals.

Responsabilidades

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
