Job summary

Indiana Professional Management Group, Inc. is seeking a full-time Case Manager to support individuals in Monroe County, IN. This hybrid role combines remote work from home with travel for community meetings, requiring residency in Monroe County and the ability to travel countywide.

You will educate on Medicaid waiver processes, build individualized plans, assist access to services, and coordinate care with a team of providers.

Qualifications

• Bachelor's degree in a human services field or related area.
• Direct experience with individuals with developmental or aging needs can substitute per policy.
• Valid driver's license, car insurance, and reliable transportation; ability to pass background screens.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an Individual support plan based on strengths, needs, goals, and desires of the individuals served.
• Help individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care with providers.
