Indiana Professional Management Group is seeking a Case Manager to provide information about Medicaid waivers, develop individual support plans, and help individuals access essential services. The role blends remote work from home with travel to community meetings within Hendricks County.

Applicants must reside in Hendricks County, Indiana and be able to travel across the county and surrounding areas for in-person meetings.

Competenze

• Bachelor's degree in psychology, sociology, social work, or another human services field, or a related degree.
• Bachelor's degree in an unrelated field with 1+ year direct experience serving individuals with intellectual/developmental disabilities or 2+ years with aging populations.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.
• Designated home office with computer, internet, and smartphone.

Mansioni

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an individualized support plan based on strengths, needs, goals, and desires of individuals served.
• Help individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care with providers as needed.

Formazione

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field

Bachelor's degree in an unrelated field with minimum 1 year direct experience with people with IDs/DD or 2 years aging population experience
