Job summary

Indiana Professional Management Group is seeking a Case Manager for a full-time hybrid role serving Monroe County, Indiana. The position requires travel within the county for community-based meetings several times a week and supports individuals through Medicaid waivers.

A home-office setup and valid driving credentials are necessary. You will educate clients on the waiver process, create individualized support plans, and coordinate with providers to meet goals while maintaining work-life

Qualifications

• Bachelor’s degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Bachelor’s degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
• Valid driver’s license, car insurance, and reliable transportation.
• Designated home office setting with computer, internet service, and smart phone.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.
