Job summary

Colorado Coalition for the Homeless is seeking a Case Manager in Denver to provide time-limited, coordinated stabilization and support to clients exiting homelessness. You will work with a collaborative team to ensure rapid exits to housing and follow-up care for the first year, coordinating medical, behavioral health, and housing services.

The role emphasizes crisis intervention, client advocacy, and linkage to benefits, with a focus on trauma-informed engagement and outcomes tracking.

Qualifications

• High School Diploma or GED required.
• Bachelor’s degree in Psychology, Social Work, Human Services, or Criminal Justice preferred.
• Basic Life Support Certification required within 30 days of employment.
• Must have and maintain Colorado driver’s license within 30 days of hire.

Responsibilities

• Manages a caseload of clients with high-service utilization and disabilities.
• Maintain positive and co-working relationships with community partners and health providers.
• Works under direct supervision within a team and supports problem-solving.
• Support clients with wrap-around care management, crisis, and engagement.
• Act as liaison between program management, client referrals, and partners.
• Respond promptly to crises, providing de-escalation and appropriate referrals.
• Develop and implement crisis and safety plans tailored to client needs.
• Mediate conflicts to maintain positive relationships.
• Track client data, engagement, and outcomes; focus on individual results.
• Transport clients as needed; accompany to appointments and outreach as required.
• Build rapport and connect clients to longer-term care and benefits.
