The People Concern is seeking a dedicated Case Manager in Los Angeles, California, to provide support for clients facing homelessness. The role involves maintaining a caseload of clients, developing supportive relationships, and completing assessments to guide treatment planning.

Qualifications

• Bachelor's degree preferred or two years experience working with homeless individuals.
• Valid driver's license and ability to transport clients.
• Demonstrated knowledge of case management.

Responsibilities

• Maintain a caseload of approximately 20 high acuity clients.
• Develop relationships with clients to facilitate independence.
• Complete comprehensive biopsychosocial assessments.

Skills

Knowledge of case management
