Inspire Together is a nonprofit organization dedicated to supporting youth and young adults experiencing homelessness, housing instability, foster care involvement, justice-system involvement, and other barriers to stability. Our programs are guided by Housing First, Positive Youth Development, Harm Reduction, and Trauma-Informed Care principles.
Position Summary
The Case Manager provides direct, client-centered case management services and serves as a primary point of contact for participants. This position supports clients in identifying their needs, developing individualized goals, accessing community resources, and working toward housing stability, employment, education, and self-sufficiency.
Key Responsibilities
- Conduct client screenings, intakes, assessments, and initial problem-solving conversations.
- Develop individualized case plans based on each participant’s needs, strengths, and goals.
- Maintain regular communication and follow-up with assigned clients.
- Connect clients with housing, employment, education, healthcare, benefits, and other community resources.
- Provide referrals and coordinate services with community partners and service providers.
- Assist clients in addressing barriers to housing stability and self-sufficiency.
- Provide outreach and participate in community events and engagement activities.
- Facilitate or support life-skills classes, workshops, and other participant activities.
- Advocate for clients with social service agencies, community organizations, and other systems when appropriate.
- Participate in case conferences and collaborate with supervisors and other program staff.
- Monitor participant progress and update case plans as needed.
- Maintain accurate, timely, and confidential case notes, records, and program documentation.
- Follow all organizational, program, confidentiality, and documentation requirements.
- Participate in staff meetings, trainings, and other assigned activities.
Qualifications
- High school diploma or equivalent; some college or a degree in social work, human services, psychology, or a related field is preferred.
- Minimum of 1 year of experience in case management, social services, housing services, outreach, or a related field.
- Experience working with individuals experiencing homelessness or other significant barriers is preferred.
- Knowledge of local community resources and housing services is a plus.
- Strong communication, organization, documentation, and problem-solving skills.
- Ability to work effectively with individuals experiencing emotional stress, conflict, or challenging circumstances.
- Ability to maintain professional boundaries while providing compassionate and respectful support.
- Basic proficiency with Microsoft Word, Excel, Outlook, and PowerPoint.
- Ability to maintain confidentiality and handle sensitive client information appropriately.
- Must be able to pass required background and fingerprint clearance checks.
Work Environment
This is a Monday–Friday, in-person position in Los Angeles. The Case Manager will work directly with participants at the program site and may participate in community outreach and events as needed.
