Job summary

Catholic Charities Eastern Washington in Spokane provides supportive housing care coordination for participants at the Catalyst emergency housing project, coordinating with partner agencies to move participants toward personal independence.

This role handles intake assessments, treatment planning, liaises with landlords, health services, and community resources, while ensuring HIPAA compliance and following COA standards.

Qualifications

• AA degree and 4 years of experience or BA in Psychology or Social Work, or equivalent from an accredited academic institution and 2 years of experience.
• Preference given to individuals with lived experiences with chemical dependency or mental health disorders.
• Knowledge of community services and a minimum of two years' experience in the direct treatment of mentally ill persons.
• Experience working with issues of mental health and poverty required.

Responsibilities

• Provide supportive housing care coordination and liaise with partner agencies.
• Assist households in finding suitable housing and act as landlord liaison during housing search.
• Coordinate with landlords and treatment teams to address tenancy issues proactively.
• Link recipients to primary care, health homes, substance abuse treatment, vocational, education, and social supports.
• Conduct intake assessments, needs assessments, treatment planning, and reassessment services.
