Job summary

Catholic Charities Eastern Washington is seeking a Care Coordinator to provide supportive housing services for clients discharging from Eastern State Hospital or qualified under Medicaid Transformation Demonstration. You will coordinate care with partners to promote independence and community integration.

Responsibilities include housing searches, landlord liaison, and linking clients with primary care and social supports, while adhering to care standards and HIPAA requirements.

Qualifications

• BA in Psychology or Social Work from an accredited institution; preference for lived experience with chemical dependency or mental health disorders.
• Knowledge of community services and at least two years’ experience in direct treatment of mentally ill persons.
• Must have a valid Driver’s License and ability to drive for work use.

Responsibilities

• Provide supportive housing care coordination for households in housing, discharge from ESH, or eligible under Medicaid Transformation Demonstration.
• Coordinate with partner agencies and treatment teams to move clients toward personal independence.
• Act as landlord liaison, coordinate background checks, and facilitate housing processes.
• Link clients to primary care, health homes, substance abuse treatment, and social supports.
