Job summary

Catholic Charities Eastern Washington seeks a dedicated OSHS team member to provide wrap-around support for clients in PSH programs. The role focuses on training, resource connections, and care coordination to stabilize individuals and families and improve health-related outcomes across multiple PSH buildings in Spokane.

The position emphasizes collaboration with healthcare providers, social workers, and program partners, following social work tenets, HIPAA safeguards, and Catholic Social

Qualifications

• Master’s degree in social work (MSW), Counseling, Public Health (MPH), Nursing, or related medical program; or scheduled to graduate within six months.
• Or enrolled in a master’s program in Social Work (MSW), Counseling, Public Health (MPH), Nursing, or related medical program with SUDP or other WA DOH cert.
• Or a master’s degree with three years of direct service experience in supportive housing or healthcare settings.

Responsibilities

• Coordinate care with OSHS staff and healthcare partners to stabilize clients.
• Review, update and help implement service plans in coordination with the ICM team.
• Maintain records documenting services and progress for each client.
