Job summary

Catholic Housing Services in Seattle is seeking a dedicated Case Manager to support residents through intake, service planning, and ongoing outreach. You will coordinate housing resources, document progress, and advocate for residents to maintain stable housing in a multi-service setting.

The role requires experience with homeless or vulnerable populations, strong communication, and ability to work in a team. Full-time with comprehensive benefits is offered.

Qualifications

• Minimum two years’ work experience with homeless, drug addicted and/or mentally ill adults.
• Ability to maintain client records with good problem solving and crisis intervention skills.
• Effective written and verbal communication in a professional demeanor.
• Experience in residential facilities or similar settings.
• Basic computer skills to maintain client records and resources.
• Ability to work independently and as part of a multidisciplinary team.
• Knowledge of mental health, domestic abuse, addiction, and homelessness issues.

Responsibilities

• Complete new resident intake and case management plans.
• Develop Housing Service Plans addressing identified needs.
• Engage residents through intensive outreach and activities.
• Provide case management including benefits, health care, mental health and other services.
• Document services and plans in resident files and HMIS.
• Assist with appointments, transportation, and home visits.
• Complete monthly/quarterly services reporting and maintain files.
• Facilitate group activities at least twice a month.
