Job summary

Hospice of the West is seeking a Full Time Hospice RN Case Manager to join our nursing team delivering hands-on hospice care, coordinating services, and guiding patients and families through the journey.

As part of the interdisciplinary team, you’ll work Monday–Friday 8 am–5 pm in the West Valley territory, bring clinical excellence, compassionate support, and education to families while ensuring high-quality end-of-life care.

Qualifications

Active RN license with no encumbrances.
Minimum 1 year RN experience in a medical-surgical/acute care setting; hospice experience a plus.
Computer proficiency, typing, and EMR experience.
Current CPR certification if required by state.
Current driver’s license and reliable transportation.

Responsibilities

Empower Through Engagement: respond to clinical referral information with care and expertise.
Collaborative Care Champion: partner with Clinical Director to assess eligibility for hospice services.
Nurturing Direct Care: provide hands-on hospice nursing care and guide patients and families.
Nursing with Heart: deliver top-notch care rooted in proven principles and techniques.
Educator and Advocate: educate patients and families to participate in care planning.
Call of Compassion: participate in call duties and be there when it matters most.
