Job summary

Hospice of the West is seeking a full-time Hospice RN Case Manager (RNCM) to join our nursing team delivering compassionate end-of-life care across the community. Monday–Friday, 8 am–5 pm, West Valley territory, with hands-on care and care coordination.

You will work with an interdisciplinary team, guiding patients and families through their hospice journey while maintaining clinical excellence, compassion, and connection. Competitive benefits and onboarding provided.

Qualifications

• Hold a current RN license.
• Minimum 1 year of med-surg/acute care nursing experience; hospice a plus.
• Must be proficient with computers and EMR systems.
• CPR certification may be required by state.
• Current driver's license, vehicle insurance, and reliable transportation.

Responsibilities

• Respond to clinical referral information with care and expertise, addressing inquiries with compassion.
• Collaborate with Clinical Director to assess eligibility and suitability for hospice services.
• Nurse with direct care, focusing on patient comfort and quality of life.
• Provide nursing care rooted in evidence-based practices to ease pain and promote comfort.
• Educate patients and families, empowering them to participate in personalized care plans.
• Share call duties as part of the team to ensure timely patient support.

Hospice of the West is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$82,000 - $94,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
