Job summary

Assisted Home Health & Hospice in Phoenix, AZ is seeking a Hospice RN Case Manager to coordinate patient care in the West Valley. The role focuses on admission criteria, pain management, and caregiver support to ensure high-quality end-of-life services.

Applicants must be a graduate of an approved nursing program with an active AZ nursing license, CPR, health clearance, and TB clearance. This is a full-time, on-site position with a strong commitment to compassionate care.

Qualifications

• Must be a graduate of an approved school of professional nursing.
• Current, active license in the state AZ Board of Nursing.
• Current CPR, health clearance, and TB clearance.

Responsibilities

• Apply admission criteria and re-certification to determine eligibility and level of care.
• Assess impact of serious illness or terminal diagnosis on patient’s needs (physical, functional, safety, psycho-social).
• Assess pain and develop an individualized pain management plan.
• Identify patient verbal and non-verbal communications patterns.
• Assess caregiver ability to meet patient needs at start of care and during care.
• Develop an individualized palliative plan of care addressing comfort and symptom relief.
