Job summary

Hospice of the West is seeking a compassionate Registered Nurse Case Manager to join our nursing team in Casa Grande. You will deliver hands-on hospice care, coordinate services, and guide patients and families through their hospice journey.

The role offers a Monday–Friday schedule, with a focus on clinical excellence and compassionate support across our community. Join a mission-driven team dedicated to dignity, comfort, and human connection.

Qualifications

Active, unencumbered RN license.
At least 1 year RN experience in medical-surgical/acute care; hospice experience preferred.
Proficient with computers and EMR systems.
Current CPR certification (as required by state).
Valid driver's license, vehicle insurance, reliable transportation.

Responsibilities

Empower through engagement: respond to clinical referrals with care and expertise.
Collaborative care champion: partner with Clinical Director to assess eligibility for hospice services.
Nurturing direct care: provide hands-on hospice nursing care and maintain patient comfort.
Nursing with heart: deliver top-notch care rooted in evidence-based practices.
Educator and advocate: educate patients and families and involve them in care planning.
Call of compassion: participate in on-call duties to support timely care.
