Qualifications

• 3 years of certified home health experience
• Experience in data analysis and statistical processes in quality reporting
• Ability to design and develop performance measures
• Excellent written and verbal communication skills
• Willingness to travel to branches (regions) as needed

Responsibilities

• Assist agency efforts toward improving clinical documentation and outcomes
• Compile trends and reports quality data in patient care, safety, risk management, infection control, outcomes and customer satisfaction
• Lead development of Performance Improvement Plans based on audit results
• Develop interventions to impact quality scores
• Review OASIS and ensure compliance with federal/state regulations
