The ideal candidate has 3 years of certified home health experience, CT licensure, and strong communication skills to educate staff and lead PI teams. Some travel to branches nationwide may be required.

Qualifications

• 3 years of certified home health experience required.
• Value Based Purchasing, PDGM, CAHPS-preferred.
• CT Licensure in field of specialty is required.
• Knowledge of federal and state regulatory requirements.
• Excellent written and verbal communication skills.

Responsibilities

• Assist with process improvement plans, record reviews and data analysis.
• Compile trends and report quality data on patient care, safety and satisfaction.
• Lead development of Performance Improvement Plans.
• Coordinate with regulatory and quality teams to ensure compliance.
• Promote evidence-based guidelines to improve clinical outcomes.
