Job summary

Affinity Care of Sarasota is seeking an MSW to implement standards of care for hospice patients and families, serving as a core member of the interdisciplinary group and providing psychosocial support based on ongoing assessments.

The role includes psychosocial assessments in patient residences, crisis intervention, care planning with patients and families, education on advanced directives, and coordination with physicians and community resources to ensure seamless care.

Qualifications

• MSW from a CSWE-accredited program.
• Minimum of one year of healthcare experience; hospice experience preferred.
• Demonstrates strong verbal and written communication.
• Must have a valid driver’s license and reliable vehicle.
• CPR certification maintained as required.

Responsibilities

• Assess psychosocial status of patients and families; communicate with the interdisciplinary group.
• Provide crisis intervention and counseling related to stress and coping.
• Educate on advanced directives and cultural considerations.
• Participate in developing individualized care plans with patient and family.
• Assist with funeral planning and financial/healthcare decisions as needed.
• Maintain accurate clinical notes and coordinate with community agencies.
