Job summary

Affinity Care of Sarasota in Florida is seeking a Medical Social Worker (MSW) to implement standards of care for hospice services. The MSW is a core member of the Interdisciplinary Group and provides psychosocial support to the patient/family unit based on ongoing assessment of needs and goals.

Duties include psychosocial assessments, crisis intervention, education on advance directives, participation in the plan of care, and coordination with the physician and team.

Qualifications

• A graduate of a Master’s program in Social Work (MSW) from a school accredited by the Council on Social Work Education
• Minimum of one (1) year experience in health care; hospice experience preferred
• Must be licensed driver with insured vehicle; CPR certification required

Responsibilities

• Assesses the psychosocial status of patients and families/caregivers related to terminal illness and communicates findings to the interdisciplinary group
• Carries out social evaluations and plans intervention based on evaluation findings; provides crisis intervention when necessary
• Educates patients and families on advanced directives and participates in development of the individualized plan of care
• Participates in interdisciplinary group meetings and documents psychosocial aspects of care
• Maintains a safe environment and coordinates with physician and team to support patient care
• Performs on-call duties and supports ongoing quality assessment and improvement activities
