Job summary

Affinity Health Management is seeking an MSW to implement standards of care for hospice medical social work services, supporting patients and families. You will be part of the Interdisciplinary Group, providing psychosocial assessment, counseling, and coordination with the care plan in the patient’s residence.

The role requires MSW credentials, hospice experience preferred, and strong communication and organizational skills.

Qualifications

• Excellent verbal and written communication abilities.
• Strong organizational and time-management skills.
• Ability to collaborate with interdisciplinary teams.

Responsibilities

• Assess psychosocial status of patients and families, and communicate findings to the care team.
• Provide crisis intervention and counseling as needed.
• Assist with advanced directives and care planning.
• Support funeral, financial, legal, and healthcare decision discussions with families.
• Attend interdisciplinary group meetings and coordinate with community resources.
