Job summary

Humana's CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and provide speech-language pathology services in patients' homes or affiliated facilities. You will participate in plan of treatment and care plans, maintain documentation, and collaborate with care teams to optimize outcomes.

The role requires ASHA CCC, at least six months of experience, a valid license and CPR, plus a valid driver’s license and reliable transportation.

Qualifications

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA.
• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Use professional skills to assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapeutic treatments.
• Observe, record and report changes in the patient's condition to supervisor/physician.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation; prepare written reports and care plans as required.
• Participate in care coordination activities and discharge planning.
• Maintain confidentiality and share information only as required.
