Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to evaluate and treat patients in their homes or facilities, under the Clinical Manager. You will develop and review treatment plans, advance rehabilitation, and enable independence for patients to regain daily activities.

The role requires ASHA CCC, at least six months of SLP experience, a current license, CPR certification, and a valid driver’s license with insurance. Travel is involved as part of home health services.

Qualifications

• Must have ASHA Certification of Clinical Competence (CCC).
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted license to practice.
• Current CPR certification is required.
• A valid driver's license with auto insurance and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent and minimize effects.
• Administer and interpret diagnostic tests and therapies including audiologic screening.
• Observe, record and report changes in patient condition to supervisor and physician.
• Provide instruction on alternative communication systems when appropriate.
• Provide counsel to patients, families and healthcare staff and maintain documentation as required.
• Participate in care coordination activities and discharge planning.
