Job summary

CenterWell Home Health, part of Humana Inc., seeks a Home Health Speech-Language Pathologist to evaluate and treat patients in their homes or facilities. You will participate in care planning, assess speech, voice, language, hearing and swallowing disorders, and administer diagnostic tests with proper documentation.

Qualifications

• Must meet the ASHA CCC-SLP requirements.
• Minimum of 6 months experience as a Speech-Language Pathologist.

Responsibilities

• Evaluate, direct and provide speech-language pathology services in the home or facility.
• Participate in development and review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and treatments.
• Observe, record and report changes to supervisor/physician.
• Provide instruction for alternative communication systems where appropriate.
• Counsel patients, families and healthcare staff and maintain documentation.
