Job summary

CenterWell Home Health, part of Humana, seeks a licensed Speech-Language Pathologist to deliver in-home therapy, evaluate and direct speech/language services, and participate in plans of treatment.

You will document evaluations, coordinate care, guide families, and support discharge planning. A valid ASHA CCC, current license, CPR, and driver’s license with reliable transportation are required.

Qualifications

• ASHA Certification (CCC) or equivalent
• Six months of experience as a speech-language pathologist
• Current and unrestricted licensure
• Current CPR certification
• Valid driver’s license and reliable transportation

Responsibilities

• Evaluate, direct and provide speech/language pathology services to patients in their home or facility
• Develop and review the Plan of Treatment and Plan of Care
• Assess and treat disorders of speech, voice, language, hearing and swallowing
• Administer and interpret diagnostic tests and therapeutic applications
• Document changes in patient condition and response to treatment
• Instruct patients in use of alternative communication methods when appropriate
• Counsel patients, families and healthcare staff; prepare reports and care coordination documentation
• Participate in care coordination and discharge planning
• Maintain confidentiality and appropriate information sharing
